[T3] Spark plug blown out of #4 cylinder

I once had one blow out after doing a service on the Fastback.  I can only
assume the plug wasn't fully tightened and worked loose.  It was idling for
quite a while before heading out for a road test.  There was no sign of
thread damage and it didn't happen again.

My '68 FI Sqbk blew out the spark plug from its #4 cylinder.  Why would this
happen?  Car was running fine just before it happened.  It probably needed a
valve job but would this affect the SP?   Just asking.
