We had exactly what you want - on a '64 Squareback which had a new door
fitted when some idiot reversed into it.  The pull-up button hole was
blanked off with a grommet. Trouble is, I sold that car in '76 and it was
getting a bit rough even then - holes in the rockers already - so it
probably wouldn't be around now, and it was in the UK anyway!

New situation !!
 Apparently the Notchback kept the 3 bolt hinge for the entire run from
62-73.  What makes the 67 unique is that the inside lock is a pull-up lock. 
So I actually need a 67 "Notch" door that has a mirror hole, pull-up
interior lock, 3 bolt hinge and early door handle with lower lock. Wow ,
needle in the hay stack. 
Looks like I'm going to have to retrofit my door after all .
