[T3] Spark Plug Glitch

T III 73 engine; standard:    This never happened before.  Took out and touched up 1, 2, 3 spark plugs...wiped and put touch of oil on the threads.  Back they went, no problem.  
  Number 4 the same...put it back in...but then the VW spark-plug socket extension would not pull out.   It wiggled, but it was jammed somehow.   Had to use a long screwdriver against the insert twisting rod to pry it out...against the tin below.  I could not pull it out by hand.

I redid the procedure to see how it went.  Plug unscrewed, it and socket wrench thing came out.  I felt around in the hole area.  Nothing remarkable.  No damage evident on the plug.   Put it back in and tightened it down...Again, the socket wrench would not come out.  Had to do the screwdriver pry trick again.Started the car...no signs or sounds of problem in No. 4 plug area.  Unexplained problem solved in unknown way.
What possible explanation?  There is a bolt and nut sticking up coming from the head into lower area of the plug access hole.   It seems solid and tight.  Could any sort of metal movement in that area cause the spark plug wrench to jam when trying to pull it out?  It tightened and un-tightened the plug alright.   And, can't see any signs of scratching etc on the wrench metal.
