That doesn't mean to extend the handle, as in using a "cheater bar."

It requires a "sideways" extension at the drive end.

With the bending lever type of torque wrench, you can just use an 
extension equal to the length between the drive and the indicator.

With a click type torque wrench it's not as simple, because you don't 
know where in the handle the torque is being sensed.  The length of 
the extension is something the torque wrench maker will have to tell 
you.
