What exactly sets the regulating voltage and how does it wear and change the
voltage? Can this be adjusted?

I suspect that diode is Ge good for a couple amps. You could replace 
it with a Shottky, but that would probably lower the charging current 
limit a bit. Before you try replacing it, you should check the 
regulating voltage. If it's low, the VR is probably not worth saving.

The diode only acts to limit the charging current. If it's open, 
there's no limit. Your symptoms don't sound like that kind of 
problem.
