I've tried that "teeth lifting" thing, previously with about 50%
success.  Trying to stabilize the visor arm in a vise and prying one
tooth at a time ... with one hand ... is (almost) impossible, for me.
You need about 4 hands and several pointed tools.   It seems the
grooves are not "screw-like" but rather individual rings on the shaft.

I guess I could cut the shaft off, below the washer and PUSH it off
... downward, rather than trying to manipulate it to PULL the washer
off in the opposite direction which it was assembled.

I did learn that the sun visor mount, shaft, spring and lock washer
could be removed  by rotating the shaft 180° and pulling the entire
assembly out from the vinyl visor .

I've searched many hardware sources and can't seem to find any
internal tooth lock washers that are made of spring steel, like the OE
items.  The lock washer teeth are also "tilted" to serve their lock
washer function, rather than the "push-on grabber" OE washers.
