They're actually a circular  cut out from the inner skin on mine, with the
one at the rear opened-out  substantially by the inner skin pressing of the
door, partially under the  door rubber. 

I've attached a photo for Jim, Bob and  John.
 
Yup, Dave's correct. I was doing a GM truck the day before, and mixed up  
what I was seeing with the 2 different styles of doors. Thanks for the 
picture  Dave, it helped me remember which was which. ;-)
